December 6, 1963
Very hard to find. Not a copy or reproduction. In phenomenal condition for its age.
Both sides of the survey are shown.
Any slight glare is from my camera flash. The survey is in fantastic shape. It was at one time long ago folded into thirds but has been stored flat for fifty years and the folds are very minor and pretty hard to discern.
There is also the tiniest of bumps to the upper left corner of the front of the survey. See pictures. What you see is what you get.
